​NOVEMBER INDOOR TOURNAMENT

• Official Team Roster or Tournament Only Roster  
• Signed Team Waiver Form (above)
• Bring passcards or player's driver's license or passport or birth certificate & picture to field in case of challenge.

MEMORIAL OR LABOR DAY TOURNAMENTS

• Official Team Roster or Tournament Only Roster Signed by League with player numbers
• Signed Team Waiver Form (above)
• Permission to Travel for out of state teams
• Risk Mgt Card or Equivalent Background Check
• Concussion & Safe Sport Cert for Coach (done online at www.michgianyouthsoccer.org)
• Player & Team Offficials ID Passcards/Passbooks (bring to field)
• Medical Release Form or Permission to Treat for each player  (bring to field)
• Concussion Form signed by parent/player (bring to field)
